Book a Birthday Party

I hereby give The Newark Museum the absolute right and permission to use photographs and video for advertising, promotional and publicity purposes. I understand that the video and photograph(s) may be used in any and all types of electronic and printed media without limitation, including, but not limited to, publications, print ads, direct-mail pieces, and such electronic media as video, CD-ROM, Internet and World Wide Web.

I hereby release the photographer and The Newark Museum, its officers, employees, agents
and designees from any and all liability for damages or losses to persons, property or property rights arising out of or resulting from the above-described use of any photograph(s) of my child that are the subject of this release.

Further, I hereby waive any right to inspect or approve the finished product or other copy that may be used in connection therewith or the use to which photograph(s) of my child may be put. I also hereby release and discharge the photographer and The Newark Museum, its officers, employees, agents and designees from any liability by virtue of any blurring, distortion, optical illusion, use in composite form or other alteration or manipulation, whether intentional or otherwise, that may occur in the processing or use of photograph(s) for the above-stated purposes.

I agree to the aforesaid use of video and photograph(s) without payment or compensation of any kind.
